Intravascular hemolysis involves the rupture of red blood cells, releasing hemoglobin and increasing free hemoglobin (FHb) in plasma. Patients with autoimmune hemolytic anemia, sickle cell anemia, or thalassemia typically exhibit mild to moderate increases in plasma FHb. Conditions such as paroxysmal nocturnal hemoglobinuria, paroxysmal cold hemoglobinuria, and unstable hemoglobinopathy may present with hemoglobinuria. Consequently, measuring plasma FHb is clinically significant for diagnosing hemolytic disorders.

Ferrous heme in hemoglobin exhibits peroxidase-like catalytic activity, oxidizing 4-aminoantipyrine and aniline analogs with hydrogen peroxide to form a purple compound. This compound shows a characteristic absorption peak at 546 nm, and its color intensity is directly proportional to the FHb concentration.
