Cellular uptake of neutral red depends on the ability of viable cells to maintain a pH gradient. Under physiological pH, neutral red carries nearly no net charge, allowing it to passively diffuse across the cell membrane in its non-ionized form. The proton gradient within lysosomes lowers their internal pH relative to the cytoplasm, causing neutral red to become ionized and accumulate inside the lysosomes. As cell proliferation increases, the number of cells rises, leading to greater total neutral red uptake. Conversely, cellular damage reduces neutral red uptake capacity. After a defined incubation period, cells are washed and lysed to release the accumulated dye for quantification. Measuring neutral red uptake thus enables assessment of cell proliferation or cytotoxicity.
